The term headache physiotherapy covers a broad range of approaches, from general cervical mobilisation and postural correction to specific assessment and treatment protocols for upper cervical dysfunction. Understanding where the Watson Headache® Approach sits within this spectrum helps clarify why it produces outcomes that general headache physiotherapy often does not.
| Clinical feature | General headache physio | Watson Headache® Approach |
|---|---|---|
| Assessment framework | Broad cervical assessment, posture, range of motion, muscular tension | Specific segmental assessment of C1, C2, and C3 using symptom reproduction as the clinical test |
| Diagnostic test | Inferred from symptom description and movement restriction | Provocation of identified segments reproduces the patient’s familiar headache symptoms |
| Treatment target | General cervical mobility, muscular tension, posture | Specific upper cervical segments confirmed as contributing to brainstem sensitisation |
| Neurophysiological basis | General musculoskeletal principles | Trigeminocervical complex, C1-C3 afferent input, brainstem sensitisation |
| Treatment structure | Variable, often open-ended | Intensive program of 5 to 6 sessions over 3 weeks |
| Session-to-session progression | Generally consistent technique | Reassessment at each session determines that session’s treatment sequence |
| Appropriate for migraine | Partially, where cervical component is identified | Yes, based on established relationship between upper cervical afferents and migraine mechanism |
This comparison is not to diminish general headache physiotherapy. Cervical mobilisation, postural correction, and muscular treatment all have clinical value for many people with neck-related head pain. The Watson Headache® Approach is the appropriate choice specifically where previous physiotherapy has produced incomplete or temporary results, or where the upper cervical spine has not been assessed at the segmental level that the approach requires.
Our practitioner attended the Watson Headache® Institute Level 1 Foundation Headache Course titled “Cervicogenic Headache and The Role of C1-C3 Cervical Afferents in Primary Headache” presented by Dr Dean H Watson PhD, Australian Musculoskeletal Physiotherapist. The course was based on the Watson Headache® Approach, a protocol for the skilled assessment and management of the upper cervical (neck) spine in Cervicogenic and Primary Headache, and related conditions in which the underlying dysfunction is sensitisation of the lower brainstem. The Watson Headache® Approach is recognised as a scientifically researched method of examination as well as a comprehensive clinical management system.
