At MyoFitness, we accept referrals under Medicare Chronic Disease Management (CDM) plans, previously known as Enhanced Primary Care (EPC). These plans are issued by your GP for individuals with chronic or complex medical conditions and allow access to a limited number of subsidised allied health sessions each year.
Important Information – Please Read Carefully
Medicare rebates are only available for physiotherapy services provided by a registered physiotherapist.
Myotherapy, remedial massage, lymphatic drainage, and other services offered at MyoFitness are not eligible for Medicare rebates, even if you have a GP care plan. Booking these services will result in full out-of-pocket payment with no Medicare rebate available.
It is the patient’s responsibility to ensure they are booking the correct service type if they wish to claim a Medicare rebate. If you are unsure, please contact our team prior to booking.

To qualify for a GP care plan, your GP must determine that you have a chronic medical condition that has been, or is expected to be, present for six months or longer. These plans are designed to support long-term management of health conditions.
Common examples include musculoskeletal conditions, asthma, cancer, cardiovascular disease, diabetes, and stroke. Your GP will assess your individual situation and determine your eligibility.
Under a GP care plan, Medicare provides rebates for up to five allied health sessions per calendar year. These sessions are shared across all allied health providers.
These sessions must be used within the calendar year (ending 31 December) and do not carry over into the following year. A new referral from your GP is required once all sessions are used or when a new calendar year begins.
Your GP referral must be valid and addressed to an appropriate allied health provider. For Medicare rebate purposes at MyoFitness, this must be a physiotherapist.
All appointments at MyoFitness are privately billed, meaning full payment is required at the time of your consultation.
For eligible physiotherapy appointments only, a Medicare rebate (currently $53.80 per session at the time of writing) can be processed after payment. This can be done instantly via HICAPS if you are using a savings or cheque account linked to your EFTPOS card.
Medicare rebates cannot be processed onto a credit card. If you are unable to process the rebate on the day, a receipt will be provided so you can claim directly via the Medicare Express Plus app or through your Medicare account.
